Shared Care in Action
Key information to be aware of when accessing Shared Care:
- Shared care with GP’s is when a GP writes your prescription for your medication. The GP will not initiate, titrate or review your medication. This would be the responsibility of Diverse Diagnostics.
- Shared care occurs if your GP is agreeable. GP’s across the UK are not required to accept shared care. If this is something you are interested in it would be important to speak to your GP.
- Some GP’s are agreeable to shared care once individuals are on a stable dose.

Your Medication Journe
So you have completed your assessment and you have a greater clarity and understanding of your mental health challenges. But what next? When it comes to mental health treatment and support medication plays an important role and many people often have many questions. At Diverse Diagnostics we want to help support you on your medication journey and answer any of your questions.
Medication Initiation Appointment
- Medication at Diverse Diagnostics is initiated by a consultant psychiatrist in a 40 miunte appointment
- This appointment is booked via phone at 0141 463 8297 or you may email info@diversediagnostics.co.uk
- Medication Initiation appointments is an opportunity for the consultant psychiatrist to review you physical health history and mental health history and advise on appropriate medication. The consultant will review your current medications, your physical observations, your GP medical summary in this appointment.
- In the medication initiation appointment, the consultant will advise on the appropriate medication, risks, side effects and give guidance on how the medication can help support you in terms of mental health treatment support.
- Sometimes a consultant psychaitrsit may request further investigations such as bloods, ECG or further medical input. This is to ensure that it is safe to start medication from a physical health point of view.
- Prior to your medication initiation appointment you will be required to provide the following:
- Pre-appointment medication information signed
- Physical observations provided – Height, Weight, Blood pressure, Heart Rate
- GP Medical Summary
- Following your appointment, the Medication Consent form is required to be signed. This form ensures that you as a patient are aware of the medication, risks and side effects.
- Patients will receive their Medication Initation letter within 3 business days following their appointment via email. This is sent to the patient and to their GP.

Private Paper Prescriptions
- Following a medication appointment patients are able to order their medication through a paper prescription.
- Diverse Diagnostics is able to provide paper prescriptions to a pharmacy of your choice across the UK.
- Patients are to let Diverse Diagnotics know the address of your pharmacy. This can be sent via email to info@diversediagnostics.co.uk
- It is recommended that you pre check with your pharmacy that your medication is in stock prior to prescription being sent.
- Paper prescriptions cost 55£ including delivery to your pharmacy directly. Payment links for the prescription are sent via email with your medication letter following your medication appointments.
- Diverse Diagnostics is unable to send prescriptions directly to patients as per our regulatory requirements.
- If you have changed your pharmacy it is important to inform Diverse Diagnostics. We are unable to send another prescription without a further prescription payment fee of 55£
- Paper prescriptions are written on set prescription writing days including M/W/F before 9 am.
- Prescriptions for controlled drugs ( ADHD medication) are written out for ONLY 30 days.
- Prescriptions can be ordered only 1 week in advance of your prescription running out
- It is a patients responsibility to order their repeat prescription. We recommend ordering 1 week in advance to ensure there is no break in medication.
- When order a paper prescription, please note this does not include the cost of the medication. There is a cost at the pharmacy for the medication which is the ‘dispensing fee’. Each pharmacy has their own cost to medication and we kindly advise patients to check with the pharmacy regarding costs.
Common Reasons For Delays in Prescription
- Patient has not provided required documentation ( Ex Physical Observations, GP medical summary)
- Patient has not paid for prescription
- Patient has not signed Pre appointment information
- Patient has not signed Medication consent form
- Patient is awaiting for documentation or feedback from clinician ( This feedback will be provided within 7 days)
Travelling with Medication
Be prepared so you can avoid delays
- Some countries require letters to bring in controlled drugs upon entry. Each individual country has their own laws/regulations around this and we advise patients to check prior to travelling
- A letter can be provided by Diverse Diagnostics stating that the patient is under the care of the clinic and what medication they are currently on.
- Please note all letters can take up to 3 business days from the time of payment.
- Additional letters have a required fee of 80£
Appointment Booking

- Patients are booked in for their next appointment following their medication appointments
- Patients will be booked in 7-10 days in advance of their prescription running out.
- If you are running out of medication you can order further medication to cover you until your appointment
- If you require an earlier review, you may contact the clinic directly and we can organise this for you.
- Patients are required to attend their medication reviews if they would like to continue with their medications
- If your appointment time does not suit please get in touch with the clinic at the earliest time so we can accommodate for your required time
- Appointments need to be paid 7 days in advance. If an appointment is not paid for then it will be cancelled. Cancelled appointments would need to be rescheduled within a week to continue with medication prescribing
- If a patient cancels multiple appointments, GP’s will be notified and further prescriptions will not be provided. This is due to regulatory guidance
When are Physical Observations Required?
- Many ADHD medication require physical observations to be completed. This includes Height, weight, Heart rate, Blood pressure.
- Physical observations provide us with an insight on your physical health and helps us understand if there is any side effects occurring
- Physical observations can be completed at home. You can order a blood pressure cuff off Amazon or any other site which can help with the blood pressure and heart rate monitoring. You may also go to a pharmacy or your GP to get your physical obs completed
- Physical observations should be provided in advance of your appointment to avoid delays in your prescription
- Your clinician may request further physical observation monitoring dependant on your clinical picture.

Our Process for Medication Reviews

- Medication review appointments are booked by our medication team at Diverse Diagnostics
- Medication reviews are booked based on your clinicians plan. Your plan can be found in your medication letters
- Medication reviews are a legal obligation for the clinic to ensure medication is safe and if it’s effective
- Medication reviews are scheduled in keeping with NICE Guidance and also based on your personal clinical information
- Medication reviews for stimulant medications generally for ADHD require medication reviews once a month until an individual is on a stable dose. Generally it takes 3 -4 appointments before an individual is on stable dose
- Once a patient is on a stable dose they will be requested for further reviews every 3-6 months depending on your clinical picture and age
- Once a medication review is completed with a clinical a medication letter is provided within 2 business days of your appointment. This letter is emailed directly to yourself and to your GP.
- If you have opted for a paper prescription, a link will be sent with your medication review letter.
- If you have opted for e prescribing the clinician will put in your order in your appointment.
- Please note prescriptions are only provided if all required clinical information is in including your physical observations
- Delays in physical observations will result in a delay in prescriptions. Clinicians will only be to review late documentation on their next working day (within 7 days
